FEES

Please call our office to inquire about the cost of different evaluations (prices range from $1,500 - $3,500)

We take all major credit cards, HSA/FSA cards, cash and checks.

12-month payment plans are available at a fee


WHAT TO EXPECT   


Your Assessment will likely include:

  • An intake session (Parent/Guardian's only if a minor is being evaluated)
  • Review of available records and consultation if necessary 
  • Testing day(s)
  • A feedback session to go over the results
  • A detailed report with the findings of the tests that were administered, a diagnosis (if there is one) and recommendations based on the findings

                                                           

Dr. Crum evaluates ages 4 - 89. 

If you share custody of a child you would like to be evaluated, all legal parents and/or guardians are required to consent to testing prior to scheduling the testing sessions. 

Please be advised that we no longer accept health insurance. Here are a few reasons why:


  • Insurance has created several limitations on the services we provide and because Dr. Crum would like to offer the best service and the most resources as possible, he can only do so by not going through insurance and being bound to their rules and regulations. 
  • We do not agree that an insurance company can decide whether an evaluation is medically necessary or not. If an insurance company deems an evaluation to not be medically necessary, the patient would end up having to pay out of pocket for the assessment.    
  • We do not agree that certain insurance CPT codes should be covered while others are denied, leaving the patient to pay out of pocket for an assessment solely based on the diagnosis. 
  • We do not agree that an insurance company should determine how much time is needed to do complete a competent evaluation. Dr. Crum's evaluations are very thorough and he spends more time on his evaluations than insurance companies believe is necessary.  
  • The amount of hours spent on unpaid or denied claims can be better spent helping patients.


At your request, we can provide you with a Super Bill that you can submit to your insurance for possible reimbursement. Please keep in mind that by using your insurance, you will be required to disclose all diagnosis codes associated with your evaluation which can remain in your health chart and/or employment records.

For Kids Foundation  

Serving as a safety net for children who fall through the cracks, For Kids Foundation provides one-time funding for the unmet needs of Northern Nevada children (from birth to age 21) when there is no other known resource available.

Our Cancellation Policy

All appointments require a minimum 24 hour notice of cancellation. All cancellations made less than 24 hours of appointment or 'no-shows' will be charged the full amount of the service or session.
